Multidisciplinary team

The Visiting Doctors Program is a collaborative effort between the Samuel Bronfman Department of Medicine and the Brookdale Department of Geriatrics and Adult Development of The Mount Sinai Medical Center.

The program has a dedicated team of social workers who assist homebound patients and their families with access to services and provide counseling and support in English and Spanish. We also have a team of nurses and advanced practice nurses who work closely with patients and families to ensure the highest quality care.  

The multidisciplinary staff includes:
